New Student Enrollment

To begin the enrollment procedure, register online at www.nsdga.com. Click on the NSDGA PORTAL and log in as a New Student. Make sure to fill out all the required forms, which include:  

- Registration Form  

- Guidance Form  

- Medical Form  

Once you have completed the online registration, please take a screenshot or provide proof of your Successful Online Registration and present this to the Registrar’s Office as part of the student requirements for enrollment.  

Step 1 - Visit the Registrar’s Office for your Initial Assessment  

Step 2 - Complete the Assessment for Preschool and Grade 1 conducted by the Admin and Guidance staff  

Step 3 - Return to the Registrar’s Office for the printing and signing of your Enrollment Forms  

Step 4 - Proceed to the Cashier to pay the Tuition fee  

Step 5 - Obtain your Admission Slip from the Registrar’s Office  

Step 6 - Have your ID picture taken and be sure to present your Admission Slip.

After completing the online registration, please take a screenshot or provide proof of your successful online registration and present this to the Registrar’s Office for enrollment.  

Step 1 - Registrar’s Office - Initial Assessment  

Step 2 - Cashier - For Grades 2 - 10: Secure Test Permit and pay Php 150.00; for Grades 11 - 12: Secure Test Permit with no payment needed.  

- After that, proceed to the Testing Center at the Guidance Office.  

Step 3 - Admin Office - Assessment by the Admin and Guidance  

Step 4 - Registrar’s Office - Print and sign Enrollment Forms  

Step 5 - Cashier - Payment of Tuition Fee  

Step 6 - Registrar’s Office - Issuance of Admission Slip  

Step 7 - ID Picture Taking - Please present your Admission Slip.

To complete the enrollment procedure for a new student, please prepare the following student requirements: 3 pieces of 2x2 ID pictures with a white background, 1 photocopy of the PSA Live Birth Certificate, the original ECCD Checklist or Report Card, and 1 long folder. Additionally, include Form 137/SF 10 from the previous school.

For the enrollment procedure, new students need to prepare the following student requirements: Pay Php 150.00 for the entrance exam, provide 3 pieces of 2x2 ID pictures with a white background, submit 2 photocopies of a PSA Live Birth Certificate, and present the original report card along with 1 photocopy. If you are an ESC grantee from a previous school, please include the ESC Certificate. Additionally, provide a Good Moral Character Certificate and 1 long folder. Don't forget to bring Form 137 from your previous school.

For new students interested in the General Academic Strand (GAS), Humanities and Social Sciences (HUMSS), or Accounting, Business, and Management, it's important to familiarize yourself with the enrollment procedure and understand the student requirements needed for admission.

For the enrollment procedure as a new student, please prepare the following student requirements: 

- Entrance Exam - No Payment 

- 2x2 ID Picture with a White Background - 3 pieces 

- PSA Live Birth Certificate - 2 Photocopies 

- Grade 10 Report Card - Certified True Copy 

- Grade 11 Report Card - Original and 1 Photocopy 

- SHS Voucher Certificate if applicable to Grade 11 VP5 

- Good Moral Character Certificate 

- SF 10 / Form 137 (from your previous school) 

- 1 Long White Folder
